Personal Information: Full name, date of birth, sex, email address and phone number, mailing/shipping address, and payment information (processed securely via Stripe — we do not store card numbers).

Health Information: Medical history, current medications, and health conditions submitted through intake forms. Lab results and bloodwork uploaded to your client portal. Consultation notes from your Nurse Practitioner or Medical Director.

Usage Information: Pages visited on our website, device type, browser, and IP address, appointment and order history.

We use your information to:

• Provide physician-supervised wellness services and fulfill medication orders
• Communicate with you about your care, orders, and appointments
• Send SMS and email updates about your program, refills, and health milestones
• Process payments and manage your subscription
• Improve our website, services, and client experience
• Comply with applicable laws and regulations

By providing your phone number and opting in, you agree to receive SMS text messages from Vtox Health & Wellness, including order confirmations and shipping updates, appointment reminders, program updates, health tips, and promotional offers.

We do not sell, rent, or share your phone number with third parties for their marketing purposes.

We do not sell your personal information. We may share your information with:

• Healthcare providers — your assigned Nurse Practitioner and Medical Director for care coordination
• Pharmacies — RedRock Pharmacy or other fulfillment partners to process and ship your medication
• Lab partners — Quest Diagnostics for processing bloodwork orders
• Service providers — GoHighLevel, Stripe, Google Workspace — all bound by data protection agreements
• Legal compliance — when required by law or court order

Vtox operates under physician oversight and handles Protected Health Information (PHI) in accordance with HIPAA. Your health information is used solely for the purpose of providing your care. Vtox is committed to HIPAA compliance and works with HIPAA-compliant service providers.

We use industry-standard security measures including encrypted data transmission (SSL/TLS), secure cloud storage, and access controls.
